Add "Affirmation of Confidentiality" section to new request form

Add the following text to the new request form
header text: "Affirmation of Confidentiality"
body text: "By completing this form and submitting the output package for review, I affirm that the requested outputs are safe for release and protect the confidentiality of data, to the best of my knowledge."

Also, having something on submit reminding requesters of their obligations re: Beth's comment:
"In submitting an Output Request – I would like to see a reminder that they are responsible for ensuring Outputs are non-disclosive and uphold the secrecy provisions of the Statistics Act. "

As a manager of the DI Program,

I want requesters to affirm the confidentiality of their output requests

so that I reduce the risk of a requester submitting an output for review that is not safe.
