Add a hard line break shortcode

Add a shortcode [br] for forcing hard line breaks when they are not otherwise included in the manuscript build. See discussion #1975.

When such a code is used, a manual line break after it for better visualisation in the editor should be ignored. I.e., duplicate line breaks should not be inserted.
