Refactor Birth form to include all fields spefified in the excel sheet

We need to refactor the existing form to have all the fields from the excel sheet.

Tech tasks:

• Refactor the form UI to follow the excel form design
• Refactor the request schema for the create endpoint to include all the new fields
• Refactor the mongo model to include the new fields

@Siyasanga , yes good to get each ticket separate. But remember, we're going to do the frontend UI for all tickets first so we can send screenshots for review, then we'll do the backend.
