refactor: Replace CSV file

The diff of this commit perhaps can/should be ignored; every line changed.
(Well, the blank line arguably didn't *really* change, but...)
The "description" column was split into two, quest name and quest noun (verb?).
Although this makes the design less "portable", I'm not aware
of other projects using it; I suppose they could just use the old version.
The formula was simplified to use VLookup; alternatively, a function is available.
Interestingly, although I expected the Excel size to shrink a bit (1+ kB)
(due to the reuse of quest strings), it really didn't significantly.
